New Patients

In order to allocate functions of hospitals and medical clinics promoted for nationwide, patients who visit this hospital for their first consultation, if you do not have a referral from another insurance-participating medical facility (primary care doctor) etc., you are required to pay a separate fee of 7,700 yen (tax included) as a medical expense for the initial consultation in addition to the ordinary consultation fee.

Even if you regularly visited this hospital in the past and your injuries or disease healed since then, or more than one month has passed since you stopped visiting this hospital on your own decision, your visit to this hospital again will be considered as your first medical visit. In this case, please bring a referral from another insurance-participating medical facility (primary care doctor) etc. This does not apply in the case of an emergency or if there are circumstances beyond your control.

* All those patients consulting for the first time are required to bring a referral from another medical facility (primary care doctor).

New Patients’ Reception Counter

Open 8:15 am ~11:00 am

Consultation form, insurance card, and referral (If you have one) are needed.

Diagnosis

Please consult with a doctor in charge while the consultation if you need it.
In the case of an agent, proxy is necessary.

After your consultation

After your consultation, the receptionist will return your patient card along with a bill for outpatient services. Submit these at Billing Counter No.3 and wait until you are called at Billing Counter No.4

Prescription

If we can provide the medication in our hospital, please bring the prescription to No.9 window.

And others, please go to any pharmacy nearby with your prescription.

Patient ID Card

All the patients ID cards are valid for every department.

Please be sure to bring it for the next time.

In case of loss, we reissue it for 108 yen. (tax included).

Making or making changes to the Appointments for your next visit

While you have the consultation, please ask your doctor in charge about the next consulting day to be arranged.

The reconsultation must be reserved in our regulation.

Because we basically do not accept making reservation on the phone. We would like to ask your good understanding for our regulations.

If you want to change the date of your appointments, please call our Medical Affairs Office.
